Passport Services


Passport Services Passport issued for one year can be extended for further Nine years on for validity extension can be submitted before six months to expire the passport.

In certain urgent/emergency cases, passports are issued for short validity of six months or more but less than the normal validity of 10 years. In such cases the remaining validity of the passport could be granted without payment of additional fee as the fee for 10 years is collected at the time of issue of short validity passport.

Damaged Passport

Requirements

 

· Application in EAP-I form. All particulars including name and signature in the application should tally with those in the lost passport??

· Damaged passport, in original, with a photocopy.??

· Five recent photos (Size 5cmx5cm) with white background and frontal view.??

· Copy of the First Information Report (FIR) from the Police authorities and its authentic legal English translation (This is required where the passport is substantially damaged or it is damaged in a fire/mishap. This document is not required if there is only a minor damage) ?

 

A letter addressed to the Consulate, from the sponsor/employer (from parent in case of a minor) of the applicant on the company’s letterhead intimating about the loss of the applicant’s passport and explaining circumstances leading to the loss of the passport. One photo of the applicant should also be pasted on the letter. Following format is suggested:??“This is‚to inform you that Mr./Os. ______________ s/o, d/o, w/o _____________ holder of Indian passport No._____________ issued at ________________ on ______________ whose photograph is pasted and stamped( company visa)/signed** below(, is working under our sponsorship. He/She has genuinely lost his/her above-mentioned passport. The passport was lost on ________________ in the following circumstances _____ (Please explain in detail). Kindly issue a new passport to him/her.**(by sponsor half on photo and half on paper with sponsors passport copy) ??

· Original with a copy of one or more of the following photo-affixed identity cards to establish identity and nationality (The original will be returned after verification):- ??(a) Identity Card (b) Labour Card (c) Health Card (d) Driving License (e) Election commission card??

Newspaper advertisement, in original, in two leading local dailies (one each in English and Arabic) as per the following format. Please attach full sheets of the newspaper and not cuttings thereof (This is required where the passport is substantially damaged or it is damaged in a fire/mishap. This document is not required if there is only a minor damage): ??“Indian Passport No.___________ dated _________ issued at ____________ and held by Mr./Ms. ___________^_ s/o, d/o, w/o ____________________ has been genuinely damaged. The passport was damaged on ____________ in the following circumstances. Finder please contact __________________,”

· A Sworn Affidavit signed by the applicant in the presence of a Consular Officer, explaining??a) The circumstances leading to the damage of the passport;??b) Details of travel on the lost passport;??c) Details of travel concessions including Transfer of Residence benefits, Foreign Exchange availed of, imports made by the applicant on the damaged passport;??d) Places of stay and work in UAE since arrival ??

· Personal Particulars Form (in Triplicate with pasted photos) in the case of passports )issued by other Passport Issuing Authorities??

· Personal appearance of applicant is compulsory to establish physical identity.??

NOTE:IN THE CASE OF APPLICANTS WHO ARE ON CASUAL VISIT TO UAE AND NOT A RESIDENT IN THIS COUNTRY, HE/SHE WILL BE ISSUED AN EMERGENCY CERTIFICATE TO RETURN TO INDIA.??

Loss of Passport

 

Incase of Passport lost, the following procedures to be completed to get issued a new passport in lieu of lost/damage.??Prepare an Arabic Letter in Company Letter Head addressing the Director of concerned Police Station to issue a Certificate of Loss.

 

Submit the letter along with the following documents:??

• Copy of lost passport?

• Two Photographs?

• Trade License Copy of the Company?

• Labour Establishment Card Copy to verify authorized signatory who must sign on the above letter.?

• Fee: Dh20 to be paid at Police Station?

• Police will issue a letter to Immigration Department to verify visa record. ??


Next Step:

?1.Take the letter along with all above attachments to Immigration Department (Jafiliya) and pay there Dh130 to take personal file print out of the person. ?

2. Take photocopy of all the attachments and go to the Black List verification counter and get seal of clearance. Then go to Investigation and Follow-up section and get seal of clearance. ?

3. Go to Dubai Courts and Public Prosecution to get clearance seal on the same paper.?

4. Come back to Police Station to get Certificate of Loss. Translate the certificate from Legal Translator and complete Consulate procedures.?



To apply for a new Passport in lieu of lost/damage the following procedures to be completed at Indian Embassy/Consulate:??

1. Application in EAP-1 Form. All particulars in the application should tally with those in the lost passport.??

2. Four copies of the Reference form to be filled & one photograph pasted on each. Two extra copies of the applicant's latest passport-size photograph. A total of ten copies of the photograph (Size: 5cmx5cm) will be required.??

3. Photocopy of the passport which was lost / damaged.??

4. Sworn affidavit regarding the loss / damage to be submitted. Affidavit to be filled in duplicate.??

5. Newspaper Advertisement, in original, in two leading local dailies (one each in English and Arabic) as per the following format. Please attach full sheets of the newspaper and not cuttings thereof. English Newspaper Ad will cost Dh94 (approx).??"Indian passport no ………………, issued at ……………. on …………… and held by Mr./Ms. …………………. 6. Copy of the police report regarding loss / damage of the passport along with its English translation.??

7. Photocopy of the Driving license / Identity / Labour / Health / Election Commission cards or any other document with a photograph as a proof of identity. Originals will be seen and returned.??

8. 9. Personal Particulars Form, in the case of Passport issued by other Passport Issuing Authorities (in duplicate).??

10. Personal appearance of applicant is compulsory to establish physical identity.??In case of damaged passports, the formalities at Sl. No. 5 & 6 above are required only where the passport was substantially damaged or it was damaged in a fire / mishap and ARE NOT REQUIRED in cases of minor damage.??


NOTE:In the case of applicant who is on casual visit to the UAE and not a resident in this country, he/she will be issued an Emergency Certificate to return to India.??

Newborn Baby's Passport

How can I apply for new born baby's passport in Dubai?

 

The following documents are required to apply for a new born baby's passport: ??

• Typed Application (EAP1)? ?

• Child Registration Form??

• Birth Certificate (English version). English version of Birth Certificate will be issued by Kuwait/Al Baraha Hospital (Tel. 04-2710000) for Dubai residents. It has to be attested from Ministry of Foreign Affairs. ??

• 4 photos (white background with dark dress)??

• Passport copies of parents with original.??

• Attested Marriage Certificate (attestation from concerned state Home Department. Eg. Trivandrum for Kerala, Chennai for Tamil Nadu). Spouse name must be endorsed in each parent's passport. If the spouse name is not endorsed in any of the parent's passport, EAPII Form also to be filled for endorsement. ??

NOTE:If the spouse name is not endorsed and Marriage Certificate did not attest from Home Department, a Joint Marriage Affidavit can be made from Indian ConsulateAttestation Centre. This can be done only if both spouses are in UAE.??

Passport in Maiden Name

Requirements

 

· Application in EAP-I form. All particulars other than name and signature in the application should tally with those in the present passport.

· Five recent photos with white background and frontal view (35mm*45mm).

· Current Passport in original with photocopy (of first, last, address page if separate page, any other endorsement pages and visa page with additional booklets if any.

· Personal Particulars Form (in Triplicate with pasted photos) if the passport is issued by other PIAs.

· Divorce Decree issued by a competent Court or Divorce order from Wakaf Board or Death Certificate issued by the Registrar of Deaths needs to be duly attested by Home Department of the concerned State Government in India.

· Deed Poll/Sworn Affidavit signed by applicant in the presence of Consular Officer.

· A passport expired more than six months ago should be supported by a Sworn Affidavit explaining the circumstances and the reasons for late renewal. Sworn Affidavit is to be signed by the applicant in the presence of the Consular Officer.

· Personal appearance of applicant is compulsory to establish physical identity

Passport in Married Name

Requirements

 

· Application in EAP-I form. All particulars other than name and signature in the application should tally with those in the present passport.

· Five recent photos with white background and frontal view (35mm*45mm).

· Present passport of both husband and wife in original with a photocopy each /f æirst, last, any other endorsement pages & page containing valid visa.

· Original and copy of Marriage Certificate issued by Registrar of Marriages duly attested by the Hoíe Department of the concerned State in India where the marriage is registered. If married abroad then the certificate needs to be attested from an Indian mission of that country.

· Deed Poll/Sworn Affidavit, in the prescribed format, signed by the applicant in the presence of the consular officer for change of name.

· Personal appearance of applicant is compulsory to establish physical identity.

· In the case of passports issued by other PIAs, Personal Particulars Form (in Triplicate with pasted photos).

Passport Renewal (India)

How can I renew Indian Passport from Dubai?

 

Documents Required:

• Original Passport with photocopy of page 1,2,33,35 and visa page and additional booklets if any. For minors (up to 18 years) parents' passport copies to be attached.

• Form EAP-I

• Personal Particulars Form (3 copies with pasted photos). Personal Particulars Form is not required for minor if his/her passport is issued in Dubai.

• Six recent Photos (not older than 6 months, no uniform) with white background with dark dress and frontal view (5cmx5cm). Ears, forehead to chin to be completely visible.

• Copy of Emirates ID, Labour Card or Health Card.

• Minors should present School ID Card, Emirates ID or Health Card.

• Signature should be done with black ink

• Time Taken: 5 working days (for Dubai issued passport/Tatkal renewal)

• Time Taken: 40 working days (renewal of Passport issued by other Passport Issuing Authorities)

NOTE: If you are married and spouse name is not endorsed in your passport, you should endorse at the time of passport renewal. To endorse spouse name, Marriage Certificate must be attested by concerned state home department and should be presented in original. If you cannot show attested Marriage Certificate, Consulate will renew your passport for one year only.

NOTE: If the applicant is on visit visa, an appointment letter from a company to be attached with the application. The letter must be attested by Chamber of Commerce (DCCI).

Surrender of Indian Passport

 

1. Application in EAP-II form.

2.Two Photos with white background and frontal view (35mm*45mm)an Nationality

3. Valid Passport in original, along with previous Indian passports if any, and photocopies of pages first two, last two and pages containing valid visa and other observations.

4. Valid foreign passport, along with previous foreign passports if any, and photocopies of pages first two, last two and pages containing valid visa and other observations.

5. Declaration Form prescribed u/s 8 of the Citizenship Act, duly attested by the Consulate General of India, Dubai (An Indian citizen with passport and valid visa to accompany the applicant for signing the Declaration Form).

6. Personal appearance of applicant is compulsory to establish physical identity.

7. Payment of penalty, if any, to be checked by the Consulate in terms of existing relevant instructions.

Tatkaal Passport

 

Emergency service for issue of passport can be availed by payment of an additional fee of Dhs.550/- under the Tatkal Scheme. Normal fee for adult Dhs.150/- and minor up to 18 years of age Dhs.100/-. The validity of Tatkal passport is generally for one year, which can be extended later without any fee
Detach Child From Mother Passport

 

Procedure of detaching child's visa from mother's passport and stamping visa on child's new passport.

Required Documents:

• Original Passport on which child's name is endorsed

• Original passport of the child

• Passport copy of sponsor

• Labour Contract/Salary Certificate of the sponsor
